OK WIC Online Application

Oklahoma WIC’s online application collects demographic, eligibility, and health/nutrition information used for certification. The application, developed using Qualtrics software, includes a HIPAA-compliant upload function for required documents and the ability for the participant or their representative to agree and provide a digital signature at the end of the survey. State WIC staff review applications and send them to an appropriate clinic for staff to contact the applicant and schedule an appointment to complete the certification. The expectation is for the received applications to be forwarded to clinics within one business day. The clinic is then expected to contact the applicant within two business days. On average 2,000 new applications are received each month. A separate version of the application is used to collect information, documents, and signatures for recertifying participants. On average 900 recertification applications are received each month.
